How does Self Cycle Surrogacy Program work

Our Intended Parents are having lots of questions how the self cycle surrogacy programs are managed by us. We do not leave the space for worries to the parents as everything is organized very accurately, smoothly and affectively. Here we would like to give the guidance how the program works. When intended parents are committed to start surrogacy program with us first of all they should select the desired dates of the program. Intended parents should note that for IVF programs we require their staying in destination country for 7 full days. The reason behind it is that before the patient is at very final stage of stimulation it is hard to identify the exact one day of egg retrieval. As each stimulation is individual and even previous stimulation history is not a guarantee of number of days is will last, we recommend the parents to be present from day 9 of stimulation including day 15. As soon as we know the dates from the intended parents we will start planning the program accordingly. From contract signing we need 6-8 weeks to have the IVF program fulfilled. Test results of the Intended Mother : In order the patient to be accepted for the IVF program Intended Mother should provide the following test results :

  • CBC/hb- Complete blood count/hemoglobin
  • Blood group and Rhesus – In case blood emergency transfusion is required.
  • HIV -Human immunodeficiency virus
  • HbsAg -Hepatitis B
  • VDRL- Syphillis
  • HCV- Hepatitis C
  • FSH (CD2-4) follicle stimulating hormone – tests for ovarian reserve ( must be measured on day 2-4 of menses / Menstrual phase)
  • LH (CD2-4)- Luteinizing hormone ( Must be measured on day 2-4 of menses / Menstrual phase)
  • AMH – Anti Mullerian Hormone
  • Estradiol Level – done on day 2 nd or 3 rd of monthly cycle / Menstrual phase
  • TSH – Thyroid stimulating hormone/thyroid function test
  • Prolactin- Measures the level of hormone prolactin produced by the pituitary gland. High levels can indicate infertility
  • Vaginal swab tests for Chlamydia to rule out any infection that may flare up during or after retrieval
  • Herpes Virus
  • Cytomegalovirus

Also we must receive the report of Antral Follicle Count (AFC) along with ovarian scan. The report should have very clear indication of how many antral follicles are visible in each ovary. It is important Antral Follicle Count to be done on day 2-4 of menses as most clear picture of ovarian reserve is given during monthly cycle. For the parents not having the monthly period, AFC must be done on first phase/ menstrual phase. Mentioned tests must be done within maximum 6 months before IVF dates. We must have the results in original on clinic header and official stamp. If the test is not in English, then we must have official English translation of the tests, with translator stamp on it. The results will be reviewed and approved by the IVF doctor and the head embryologist of the clinic and we will give the approval that the patient is accepted for the program with us. Test results from the Genetic father . Before we sing the contracts Intended Father should provide us with the following test results :

  • B Hepatitis
  • C Hepatitis
  • HIV
  • Syphilis
  • Complete sperm analyses ( sperm analyses should include Sperm morphology , Sperm count, Sperm Motility and Sperm Volume details)

Mentioned tests must be done within maximum 6 months before IVF dates. We must have the results in original on clinic header and official stamp. If the test is not in English, then we must have official English translation of the tests, with translator stamp on it. The results will be reviewed and approved by the IVF doctor and the head embryologist of the clinic and we will give the approval that the patient is accepted for the program with us. Options for preparation of the Intended Mother for self cycle: When the parents are stimulated, they have two options: they can either get stimulated in local IVF clinic and come to us on day 9 of stimulation and have the egg retrieval done in our clinic or alternatively be completely stimulated in our clinic. When stimulation takes place completely with us, Intended Parents should calculate their staying with us around 17 days. Parents arrival : Intended mother is supposed to be on contraceptive pills ( BCP) 3-4 weeks prior to starting the stimulation. Pills should be started on day 3-4 of previous month menses. That helps us to synchronize the parents’ menses with the surrogate mother’s menses and start their stimulation simultaneously. Based on the dates that is set for the IVF, medical coordinator informs the parents when to take last contraceptive pill and start stimulation on day 2 nd of menses. For the Intended Mother who does not have menses we advice to visit the doctor and do the ultrasound scan of the ovaries. Scan identifies when woman has ovulation phase or menstrual phase. If the lady is at ovulation phase, contraceptive pills must be started 15-17 days after ovulation. If woman is on menstrual phase this is the right timing to start taking the contraceptive pills. If the parent is stimulated completely with us, we require them to arrive three days after taking the last contraceptive pill. First visit will be arranged by the IVF doctor and stimulation will be started based on the IVF doctor suggestion and prescription. In case the intended parents start stimulation in their local IVF clinic, they should follow the following instructions:

  • Send the stimulation report on day 1-5- 8 of stimulation. The report should include the ovarian scan and follicle report in each ovary. It should be clearly written how many follicles are in each ovary and what are the sizes of the ovary.
  • On day 1 and 5 do the Estradiol test and send to the medical coordinator.
  • On day 7 do the Complete Blood count and Blood coagulation test and send to the medical coordinator.

This is very important to be provided in timely manner as our IVF doctor needs to be fully involved into the stimulation progress and prepare the surrogate mother based on the stimulation response of the parent. Intended Parents should arrive to us on day 9 of stimulation. Egg Retrieval / Sperm collection in the clinic: When the final checkup is done IVF doctor will tell the parents when they need to present in the clinic for egg retrieval and sperm collection. IVF takes place on a very same day. Parents should be aware that the abstinence ( no sexual intercourse) is required for 3-5 days before sperm collection. Longer than 5 days is not recommended as the sperm start dying off and is not good for IVF procedure.

3 days after or 5 days after IVF embryo transfer takes place to the surrogate mother.
